Qualifications

• 1–3 years in a care team or patient navigation role preferred.
• Healthcare experience preferred.
• Key competencies include empathy, problem solving, communication, active listening, and tenacity.
• Self-motivated with the ability to manage time appropriately.
• Experience with MS Office products (Outlook, Word, Excel).
• Minimum associate level education preferred; high school diploma or equivalent required.
• 1 year of medical knowledge preferred.
• Dependable transportation.

Responsibilities

• Enhance the overall patient experience and ensure patients receive the necessary support and resources.
• Accompany patients to benefit renewal appointments to advocate for the patient and program.
• Visit patient homes and update patient care.
• Effectively communicate and provide information to patients, caregivers, and physicians.
• Understand HIPAA and maintain confidentiality.
• Display a warm and friendly attitude.
