HarmonyCares is seeking a Patient Health Coordinator to support care coordination for panel patients within a collaborative Integrated Care Team. You will help manage transitions, coordinate referrals, and document interactions in the EHR while advocating for patients and families.

The role emphasizes strong organizational skills, attention to detail, and excellent communication to ensure high-quality, patient-centered care in a fast-paced home-based setting.

Qualifikationen

• High school diploma/GED required.
• 2+ years of care coordination or healthcare experience.
• Familiarity with Medicare, Medicare Advantage and Medicaid insurances and programs.
• Must be well-organized, detail-oriented, and able to multi-task in a demanding environment.
• Strong interpersonal communication and active listening skills.
• Ability to adapt to changing community needs and program environments.
• Critical thinking and sound judgment.
• Strong time management and patient advocacy.
• Self-directed with problem-solving skills.
• Knowledge of basic medical terminology.

Aufgaben

• Assist with care coordination for paneled patients.
• Collaborate with cross-functional teams to meet objectives.
• Coordinate in-market activities, relay messages, process referrals and prior authorizations.
• Monitor interventions and adjust care plans as needed in collaboration with the care team.
• Advocate for patients and share assessments during High-Risk Huddles.
• Outreach to recently discharged patients to close care gaps.
• Document all patient interactions in the EHR promptly.
• Communicate clinical information to patients and caregivers as directed.
• Work as part of a team to achieve quality outcomes and patient experience.
