Giving Home Health Care, LLC in Paducah, KY seeks a full-time Patient Coordinator to join our Case Management team. You will manage a caseload of 40–50 patients, coordinate care among patients, families, physicians, and the DOL, and remove barriers to timely services.

The ideal candidate has 2+ years in healthcare case management or care coordination, strong communication and organizational skills, and the ability to travel locally to patient homes and offices.

Formación

• Minimum of two years in healthcare case management, care coordination, or related field.
• Strong communication, relationship-building, organization, and problem-solving abilities.
• Ability to manage a patient caseload and coordinate with multiple stakeholders.

Responsabilidades

• Manage a caseload of ~40–50 patients, serving as primary contact throughout the patient journey.
• Coordinate services and communication among patients, caregivers, physicians, and internal teams.
• Monitor progress, complete follow-ups, and identify barriers to care or satisfaction.
• Advocate for patients with the DOL through calls and other advocacy tasks.
• Complete onboarding, education, outreach, home visits, and appointment support as needed.
• Maintain timely documentation, protect confidentiality, and comply with HIPAA and policies.
