Zusammenfassung

Giving Home Health Care, LLC seeks a Patient Coordinator to bridge care teams, coordinate in-home services, and support patient needs across our care continuum. You will conduct home visits, manage patient documentation, and collaborate with nurses and caregivers to ensure compliant, high-quality home health care.

Role requires 2–5 years in customer service/case management, NM driver’s license, and ability to travel frequently. Competitive benefits and mileage reimbursement are provided.

Qualifikationen

• 2–5 years of customer service and/or case management experience, preferably in healthcare.
• High school diploma or equivalent.
• Valid NM driver’s license, auto insurance, and reliable vehicle for visits.
• Ability to travel 3–4 days per week and up to 1,500 miles/month.
• Computer literacy with tablets and laptops; proficient in MS Office.
• Strong written/verbal communication and organizational skills.
• HIPAA compliance and patient confidentiality commitment.
• Background check required by NM Department of Health.

Aufgaben

• Serve as liaison between patients, caregivers, clinical staff, and internal departments.
• Conduct home visits to initiate services and gather compliance docs.
• Advocate for patients at physician appointments and ensure DME arrangements.
• Manage a caseload of 50+ patients with docs and renewals.
• Provide training on DME and coordinate with nurses and HHAs.
• Update documentation in Salesforce and EMR within 24 hours.
