Giving Home Health Care, LLC in Albuquerque, NM is seeking a Patient Coordinator to serve as liaison among patients, families, clinicians, and internal teams, ensuring clear communication and coordinated home care. You will conduct home visits, manage 50+ patient caseloads, assist with assessments, train on DME, and ensure documentation in Salesforce/Axxess within 24 hours.

The role requires 3–4 days of local travel weekly, mileage reimbursement, a valid NM driver’s license, and a flexible

Qualifications

• 2–5 years of customer service and/or case management experience in healthcare or home health.
• High school diploma or equivalent is required.
• Valid New Mexico driver's license, current auto insurance, and reliable vehicle for patient visits.
• Ability to travel 3–4 days per week (up to 1,500 miles/month).
• Computer literacy; comfortable using tablets and laptops as daily tools.
• Proficient in MS Office (Outlook, Word, Excel, PowerPoint) and basic reporting.
• Strong written and verbal communication, interpersonal, and presentation skills.
• Excellent organizational and time management skills with ability to manage a high-volume caseload.
• Understanding of HIPAA and patient confidentiality.
• Flexible availability outside normal business hours; background check required.

Responsibilities

• Serve as primary liaison between patients, caregivers, clinical staff, and internal departments for clear communication throughout care continuum.
• Conduct home visits to build patient relationships, initiate homecare services, and collect compliance documentation.
• Advocate for patients at physician appointments and coordinate home care and DME arrangements.
• Manage caseload of 50+ patients and coordinate with families and providers on medical documentation and renewals.
• Ensure patients have necessary DME and provide training to patients, aides, and nurses.
• Perform regular phone check-ins with nurses and home health aides to coordinate care.
