Job summary

WelbeHealth is seeking an Occupational Therapist to join our interdisciplinary team and advance patient-centered care for participants in our program.

You will conduct OT assessments, participate in IDT care planning, and deliver interventions to improve independence in ADLs, with a focus on safety and long-term outcomes.

Qualifications

• Bachelor’s degree in occupational therapy; master’s preferred.
• Valid license to practice Occupational Therapy (New Jersey) or eligibility.
• CPR and First Aid certification within 180 days of hire and maintain thereafter; certification on hire preferred.
• Ability to work in an interdisciplinary team and coordinate with IDT members.

Responsibilities

• Conduct OT assessments in the facility and home to define participant needs and goals.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document interventions as agreed in care plans.
• Provide active and passive exercise, muscle re-education, gait, and functional ADL training.
• Train Therapy Assistants to implement Functional Maintenance Programs (FMP).
