Responsibilities

• Provide basic patient care including comfort, vital signs, hygiene, and mobility support.
• Serve as liaison between patient and nurse to report changes.
• Deliver high-level customer service to patients, families, visitors, and staff.
• Document patient care accurately and promptly.
• Participate in hand-off reporting of patient and department needs.
• Perform delegated tasks after competency validation.
• Assist mobilization using safe transfer techniques.
• Encourage patient independence in daily tasks.
• Individualize care based on patient interests.
