Responsibilities

• Provide basic patient care including care and comfort, vital sign measurement, personal care and hygiene, and assist with mobility.
• Act as liaison between patient and nurse to report changes and/or concerns.
• Provide high-level customer service to patients, family, visitors, and employees.
• Document patient care accurately and timely.
• Participate in hand-off/report of pertinent information upon arrival and departure.
• Perform delegated tasks once competency is validated.
• Mobilize patients using therapeutic transfer techniques to foster independence and safety.
• Encourage patients to complete tasks independently and tailor care to patient interests and hobbies.
