Qualifications

• Must possess excellent communication skills, the ability to interact well with a diverse group of individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.
• Must have at least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.

Responsibilities

• Manages schedules for all patients and edits schedules for agents calling in sick, ensuring timely reassignment.
• Initiates infection control forms and handles documentation routing.
• Back up during lunch hours and busy times; assists with field calls and weekly case conferences.
• Maintains hospitalization log and coordinates notes with staff.
• Schedules visits, add-ons, and ensures supervisory/appropriate visits are set.
• Runs scheduling reports and prepares weekly schedules for Branch Director review.
• Verifies visit notes and assists with patient transfers between branches.
• If clinical, may perform patient visits and participate in on-call rotation.
