Job summary

Qualifications

• High school graduation and 2 years of experience supporting patients with opioid use disorder via telehealth.
• Bachelor’s degree in related field preferred.
• Experience with research projects or data collection is a plus.
• Proficiency with Microsoft Word, Excel, and PowerPoint; strong communication skills.

Responsibilities

• Monitor hotline and communicate with on-call providers.
• Conduct eligibility screenings and registrations for patients.
• Coordinate care for harm reduction and OUD treatment referrals.
• Enter screening and linkage-to-care data into databases with accuracy.
• Maintain patient records in accordance with HIPAA and hospital policy.
• Collaborate with medical providers, nurses, and social workers; provide outreach and education.
