Job summary

Mount Sinai Health System is seeking a Program Coordinator to enhance patient-centered care by identifying and engaging patients with care gaps and coordinating preventive and chronic care services.

You will collaborate with physicians, APPs, nurses, and operations to facilitate screenings, tests, and follow-ups, and you will help patients navigate services while ensuring timely documentation and follow-up across multiple clinics.

Qualifications

• Bachelor's degree or equivalent; experience in ambulatory care is beneficial.
• 1–2 years administrative experience preferred but not required.

Responsibilities

• Conduct proactive outreach to patients identified by physicians or care teams for preventive or chronic care needs.
• Educate patients on completing screenings, tests, examinations, and follow-ups.
• Assist patients with scheduling appointments, tests, or services.
• Provide scheduling instructions and contact information when direct scheduling is unavailable.
• Document outreach, responses, and services accurately in the medical record or population health platform.
• Escalate clinical questions to the appropriate team while staying within scope.
