SNFout is seeking a Patient Care Coordinator in Bay Shore, NY 11706 (Bay Shore, NY 10023 area) to staff patients with reliable caregivers and monitor utilization. The role requires proactive communication with patients, families, MLTCs, hospitals, and internal teams to minimize gaps in care.

The ideal candidate will manage a high-volume caseload, maintain accurate records, and escalate urgent issues when needed.

Qualifications

• Strong verbal and written communication skills.
• Experience in home care, healthcare coordination, scheduling, or related field preferred.
• Successful candidates will handle high-volume caseloads and maintain timely documentation.

Responsabilités

• Staff patients with reliable caregivers aligned to the patient’s schedule and care plan.
• Address staffing gaps and caregiver changes to prevent service interruptions.
• Monitor utilization and ensure hours are properly staffed and used.
• Communicate with patients, families, hospitals, MLTCs, and payors as needed.
• Maintain patient schedules and records in the agency systems and ensure timely follow-up.
