Job summary

HCA Florida Bayonet Point Hospital in Hudson, FL is seeking an experienced Patient Care Navigator. You will guide patients through the health care system, assist with access issues, and coordinate with service providers to improve outcomes.

The ideal candidate holds an Associate degree in health care, RN license in Florida, and BLS, with at least one year of clinical experience. You will work with physicians and the care team to support patients and families across care settings.

Qualifications

• Associate degree in health care field; Bachelor’s degree preferred.
• Minimum 1-year clinical speciality area experience required.
• Currently Florida State Registered Nurse license.
• Current BLS (Basic Life Support).
• Critical thinking, service excellence and good interpersonal communication skills; ability to read/comprehend written instructions and follow verbal instructions; PC skills.

Responsibilities

• Serve as a single point of contact for referring physicians, patients and caregivers to ensure access to clinical and support care services.
• Assist patients in understanding their diagnosis, treatment options, and the resources available.
• Assist patients and families in establishing a support network and addressing physical, social, and emotional needs.
• Communicate effectively and work with physicians and/or other members of the interdisciplinary team in meeting patient needs.
