Qualifications

• Current and unrestricted RN license in Florida.
• Graduate of an accredited nursing program.
• 2-3 years of clinical nursing experience, preferably in care coordination or oncology.
• Strong knowledge of care coordination and patient education.
• Excellent verbal and written communication; strong organization.

Responsibilities

• Serve as primary point of contact for patients and families throughout care.
• Coordinate appointments, diagnostics, treatment, referrals, and follow-ups.
• Assess clinical, educational, emotional, financial, and psychosocial needs.
• Provide clear education on diagnoses, plans, procedures, and resources.
• Collaborate with physicians and care teams to coordinate care.
• Identify barriers to timely care and develop solutions.
• Coordinate referrals to specialists and community resources.
• Monitor progress and ensure plan completion.
• Maintain accurate documentation in the EHR.
