Job summary

HCA Florida Bayonet Point Hospital in Hudson, FL is seeking a Clinical Rehab Specialist (PRN) to support inpatient rehab admissions and coordination with care teams. You will assess eligibility, communicate program details to patients and families, and help arrange transfers while collaborating with Rehab Medical Director and case managers.

A strong clinical background in rehab is required with BLS and applicable licensure.

Qualifications

• Basic Cardiac Life Support (BLS) must be obtained within 30 days of employment start date.

Responsibilities

• Assist and coordinate intake and pre-admission screening processes; document rehab admission criteria within timeframe.
• Perform on-site and off-site clinical evaluations to assist Rehab Medical Director in establishing patient eligibility for admission.
• Provide information to patient/family/caregiver about inpatient rehab and program features; communicate expectations and conduct tours of the rehab unit.
• Obtain information relating to patient resources and healthcare insurance benefits.
• Demonstrate rehab admission criteria knowledge to identify and qualify rehab patients.
