The University of Kentucky's Division of Infectious Diseases is seeking a Navigator to join the Bluegrass Care Clinic and Ryan White Programs. You will support patients and families to navigate appointments, services, and schedules within the division of Infectious Diseases, coordinating care among physicians, clinic staff, and care providers to improve treatment plans.

This role also supports medical case management by triaging walk-in clients, coordinating services, arranging transportation

Qualificações

• Use of electronic medical records and patient data privacy.
• Strong problem-solving and communication skills.
• Experience with community-based resources for patients.

Responsabilidades

• Coordinate appointments, services, resources, and care schedules for patients and families.
• Triages walk-in clients and coordinates Ryan White case management services.
• Provide transportation assistance and basic needs support according to protocol.
• Assist with enrollment and recertification processes and refer to other teams as needed.
