Job summary

The Division of Infectious Diseases at the University of Kentucky is seeking a Navigator to join the UKHC team in Lexington. The Navigator will help patients and families navigate appointments, services, and resources across the care continuum, coordinating with physicians, clinic staff, and care providers to overcome barriers and optimize treatment plans.

The role supports the Ryan White medical case management team, triaging walk-in clients, coordinating case management services, and assisting

Qualifications

• Use of electronic medical record systems.
• Good problem solving skills.
• Excellent communication and customer service skills.
• Knowledge of and experience with community resources for the identified patient population.

Responsibilities

• Guide patients and families to navigate appointments, services, resources, and schedules within Infectious Diseases.
• Coordinate walk-in triage and enroll in case management as needed.
• Provide transportation assistance and basic needs support per protocol.
• Assist with enrollment and recertification processes and refer clients to other teams when appropriate.
