Job summary

Patient Engagement Advisors is seeking a full-time Logistics Transition Specialist to work onsite at AdventHealth Wesley Chapel and in the local area, handling logistics courier duties and patient care support. Schedule is Monday–Friday, 9:00 AM–5:30 PM, onsite at AdventHealth Wesley Chapel and surrounding area.

The role focuses on delivering medications, nutrition kits, and other items, coordinating patient services, and ensuring smooth discharge and follow-up processes.

Qualifications

• High School education or equivalent required.
• Associates or Bachelor’s degree may be considered if healthcare experience is absent.
• Retail pharmacy experience preferred.

Responsibilities

• Deliver medications and supplies to patient bedside, home, or another hospital location.
• Coordinate services (transportation, nutrition, medications) based on patient needs.
• Document social determinants of health and patient indicators in systems.
• Utilize PNS and other apps to log activities and meet KPI goals.
• participate in rounds/huddles and support clinical teams.
• Ensure compliant handling and storage of medications and supplies.
