Job summary

Patient Engagement Advisors (PEA) seeks a full-time Logistics Transition Specialist to join the AdventHealth Tampa team onsite. This role involves delivering medications, nutrition kits, and patient care items to bedside or home while coordinating with care teams.

The ideal candidate has high school education, healthcare exposure, and strong interpersonal skills. You will work Monday–Friday, 9:00 AM–5:30 PM, with on-site duties at AdventHealth Tampa and local area travel as needed.

Qualifications

• High School education or equivalent required.
• Healthcare exposure or experience preferred.
• Must be 18 years or older; valid driver’s license may be required.

Responsibilities

• Secure delivery of medications and patient care items to bedside or home.
• Coordinate with clinicians and pharmacists to ensure affordable, timely delivery.
• Document activities in patient navigation/PHI-compliant systems.
• Participate in rounds/huddles and support hospital care teams.
• Meet KPI targets and maintain accurate hand-off procedures.
