Job summary

Patient Engagement Advisors seeks a Transition Specialist to help patients move from hospital to next care stage, onsite at AdventHealth Carrollwood. You will ensure discharge education and resource access, and may perform Logistics Courier duties in the local area, including medication and kit deliveries.

The role requires strong patient interaction, coordination with care teams, and use of PEA software to maximize savings and support programs.

Qualifications

Patient Engagement Advisors (PEA) is seeking a full-time Transition Specialist to work onsite at AdventHealth Carrollwood. This position also performs duties as an alternate Logistics Transition Specialist to work within the local area while performing Logistics Courier duties.

Schedule: Monday - Friday 8:00 AM - 4:30 PM, with availability to work until 5:30 PM, as needed on-site at AdventHealth Carrollwood and in local area while performing Logistics Courier duties.

PEA will provide vehicle for use of logistic courier duties only.

Job Details

As a Transition Specialist, your role is to set patients up for success to transition out of the hospital into their next stage of care. The Transition Specialist will ensure the patient is provided services, is educated on their discharge requirements (medications, follow-up care, nutrition, transportation, assistance programs, etc.), and is equipped with the resources necessary to be successful in their next stage of care. The Logistics Transition Specialist is responsible for delivering medications, nutrition, product kits and other patient care-related items as needed to home or other hospital locations for handoff.

Here are some of the tasks you will perform to assist in this transition:

• Schedule patient’s transportation as needed
• Deliver discharge prescription medications to patient’s bedside
• Assist patients with medication affordability
• Coordinate home meal delivery service for patients
• Connect patients to community resources as needed
• Deliver medication, nutrition product kits and other patient care-related items to the patient’s home or another hospital location for handoff
• Collect and document social determinants of health information for every patient to whom services are offered (to better understand patient needs and provide optimal services)
• Provide pick-up and delivery of prescriptions and medical supplies accurately and timely
• Ensure proper handling and storage of all medications and supplies
• Obtain signature of person receiving the delivery
• Collect payments; comply with payment collection related policies and procedures
• Utilize PEA software and assistance programs (coupons, vouchers, discount cards) to provide optimal savings/value for patients
• Utilize Countdown to Discharge (C2D) and other applications appropriately to document completed activities and fulfill patient needs
• Meet KPI "Scheduled" goals and ensure follow‑up appointments are documented in C2D
• Actively participate in hospital-specific rounds/huddles to obtain critical information and contribute as a collaborative member of the clinical care team/hospitalists (e.g., communicate available medication programs, unrealized referral opportunities, etc.)
• Submit one "Wow Story" based on PEA Leadership’s requested intervals, to Supervisor describing exceptional patient care experience provided

Pharmacy Operations / Logistics Courier

• Works with prescribing clinicians and pharmacists to ensure affordability and successful delivery of medications per Standard Operating Procedures

Requirements

• High School Education or equivalent required
• Must be 18 years old or older
• 6 months pharmacy technician (retail preferred) OR 1-year medical assistant / patient care technician / patient care coordinator / clinic front desk associate / nursing assistant / home health aide / other healthcare or medical experience required
• Retail pharmacy experience strongly preferred
• Other related healthcare, hospital, or medical experience will be considered
• Associate and/or Bachelor’s degree will be considered in the absence of healthcare experience
• Ability to report to work onsite as scheduled; ability to be on feet – standing, walking, and/or climbing stairs 8‑10 hours daily
• Valid driver’s license in the state of employment with a clean driving record
• Ability to receive required vaccinations (as required by PEA or hospital) unless approved for exemption

• Ability to work a flexible and/or rotating schedule, including nights, weekends, and holidays; team members may be required to work during inclement weather (as directed by PEA and facility guidelines)

Salary range: $18-19/hour
