Job summary

Patient Engagement Advisors (PEA) is seeking a full-time Transition Specialist to work onsite at AdventHealth Heart of Florida, with duties as Logistics Transition Specialist in the local area. The role includes coordinating patient transport, delivering medications, and ensuring a smooth discharge process to support post-hospital care.

Qualifications

• High School education or equivalent required.
• 18+ years old and valid driver's license.
• 6 months pharmacy technician (retail preferred) OR 1-year medical assistant / patient care tech / clinical front desk experience.
• Retail pharmacy experience preferred.
• Ability to work onsite 8–10 hours daily on feet.

Responsibilities

• Schedule transportation and deliver medications to patients.
• Deliver discharge medications and assist with affordability.
• Coordinate home meal delivery and connect patients to resources.
• Deliver medications and supplies to homes or other locations.
• Document patient information and activities in PEA systems and C2D.
