Job summary

Patient Engagement Advisors (PEA) is seeking a full-time Transition Specialist to work onsite at AdventHealth Sebring. This role also serves as an alternate Logistics Transition Specialist delivering medications and patient care items within the local area.

You will help patients transition from hospital to next care stage, educate on discharge requirements, coordinate transportation, medication delivery, and connect patients with community resources.

Qualifications

• High School Education or equivalent required.
• Must be 18 years old or older.
• 6 months pharmacy technician experience or 1 year in healthcare-related roles.
• Retail pharmacy experience preferred.
• Associates and/or Bachelor's degree will be considered in absence of healthcare experience.
• Valid driver’s license with clean driving record.
• Ability to report onsite as scheduled; able to stand 8–10 hours daily.

Responsibilities

• Schedule patient transportation as needed.
• Deliver discharge medications to patient bedside.
• Assist patients with medication affordability.
• Coordinate home meal delivery for patients.
• Connect patients to community resources as needed.
• Deliver medications, nutrition products, and other items to home or hospital locations.
• Document social determinants of health information for patients served.
