Job summary

Qualifications

• High School Diploma or GED required.
• 1+ years of customer service experience.
• Basic proficiency with Microsoft Excel, Word, Outlook, and PowerPoint.

Responsibilities

• Outreaching to new patients to welcome them to the practice and scheduling their first two appointments.
• Identifying patients overdue for preventive care and re-engaging them.
• Communicating the importance of value-based care and partnering with patients to improve health outcomes.
• Promoting and assisting with MyChart sign-ups to improve direct communication.
• Contacting new members or those new to Medicare to introduce Optum and their PCP; knowledge of Value-Based Care.
