Job summary

AccentCare, Inc. is seeking a Patient Care Liaison for our Brooklyn office to coordinate home care services and client relations.

This on-site role requires strong bilingual communication (English/Spanish), familiarity with insurance verification, and collaboration with care teams. The position offers full-time hours, a Monday-Friday 9:00am-5:00pm schedule and a focus on maintaining seamless patient care across plans and providers.

Qualifications

• High School or GED required.
• Bi-Lingual English/Spanish required.
• Previous home health care or home care experience highly desirable.

Responsibilities

• Manage relationships with insurance payers and MLTC (Managed Long-Term Care plans).
• Verify patient insurance eligibility, benefits, and coverage.
• Secure and track service authorizations for home care services.
• Resolve billing, claims, and reimbursement issues.
• Stay informed of payer policies and regulatory requirements.
• Collaborate with internal teams to ensure uninterrupted patient care.
• Monitor authorization and reimbursement activity for process improvements.
• Support community outreach and health plan marketing activities.
