Job summary

Summit Orthopedics in Minnesota is seeking a Patient Coordinator to manage clinical support coordination and scheduling follow-ups and surgeries. You will facilitate cross‑team communication to ensure smooth patient care under the Practice Development Manager.

This full‑time float position runs Monday–Friday 7:30 AM–4:00 PM with primary locations TBD and possible coverage at outreach sites. Education: High School required; Associate degree preferred; 2 years of related experience highly valued.

Qualifications

• High School diploma is required; Associate degree preferred.
• 2 years of related experience in medical administrative or care coordination.
• Ability to manage multiple schedules and prioritize tasks.
• Strong communication across care teams.
• Knowledge of FMLA and other patient forms is a plus.

Responsibilities

• Answer and respond to patient phone calls, manage team voicemail and email, and triage patient phone calls on a daily basis.
• Manage physicians’ daily schedules, clinic and surgery schedules.
• Support coordination of patient care between team members, other service line teams and/or other Summit departments.
• Communicate information from physicians and other care providers to patients and other staff, in support of patient care.
• Enter physicians’ orders into electronic medical record.
• Complete standard non-complex patient forms such as FMLA, Short Term Disability, Workers Compensation, and other requests.
• Act as immediate back-up to other teams and assist with voicemail, email and task lists as needed.
• May be responsible for rooming patients and initiating visits, as directed.
• Perform other duties as assigned.
