Job summary

AdvantageCare Physicians (ACPNY) is hiring a Care Team Associate I to support care teams with non-clinical and administrative tasks focused on population health. The role reports to the Practice Administrator and collaborates with the Nurse Manager.

Responsibilities include patient check-in/out, appointment scheduling, insurance verification, and coordinating internal/external referrals. Strong customer service, data accuracy, and EMR proficiency are required.

Qualifications

• 3-5 years’ experience in customer service/care coordination in an ambulatory care setting.
• Experience and ability to navigate an EMR.
• Strong communication and teamwork skills.

Responsibilities

• Provide general nursing care to patients and coordinate care under supervision.
• Greet patients, manage wait times, and assist with registration and checkout.
• Answer calls professionally and schedule appointments in the system.
• Verify insurance coverage and collect payments as required.
• Coordinate referrals and follow up on pre- and post-referral matters.
• Maintain accurate patient data in electronic systems.
