Your Health in Augusta, GA seeks a Clinical Coordinator to support clinical care for patients in daily office operations. This full-time, salaried role runs 8:00 AM–5:00 PM, Monday–Friday, with responsibilities including chart prep, scheduling, and care-management logging.

Experience in a similar role is valued; the position emphasizes fast-paced, value-based care and close collaboration with providers and care teams in an office setting.

Qualifications

• Must have CMA or licensed nurse with active license in the state of employment.
• Minimum of 1 year experience in a healthcare setting preferred.
• Must hold and maintain current CPR certificate.
• High school diploma or equivalent required.
• Strong interpersonal and communication skills.
• Basic computer knowledge.
• Must be able to travel between offices and be a licensed driver.

Responsabilités

• Review and organize patient medical records in preparation for visits.
• Ensure all necessary documentation, test results, documents, diagnoses, and relevant information are accessible for the provider; use Navina to ensure these are added, completed, and/or updated.
• Ensure confidentiality and adhere to privacy regulations; verify active consent forms and alignment paperwork are completed and in the patient’s chart.
• Reconcile medications.
• Collaborate with care team to schedule and coordinate patient appointments with the Company’s provider(s).
• Interact with patients via email, patient portal, telephone, text, in-person to gather information, schedule and confirm appointments, and provide pre-appointment instructions.
• Use communication methods such as phone, email, AthenaText, Microsoft Teams.
• Appropriately and accurately document and log care management activities.
