Your Health in Rock Hill seeks a Clinical Coordinator to support clinical needs for local patients. The role focuses on chart preparation, scheduling visits, logging care management activities, and supporting providers and patients.

This full-time, 8-hour salary-based position runs Monday to Friday, 8:00 AM to 5:00 PM, with travel between offices and facilities as needed. CMA or nursing licensure, CPR, and strong communication are required.

Qualifications

• Must be CMA or licensed nurse in good standing.
• CPR certification required.
• Strong interpersonal and communication skills.
• Ability to manage multiple responsibilities and adapt.
• Ability to read and communicate effectively, basic computer knowledge.

Responsabilités

• Review and organize patient records for visits.
• Ensure documentation and test results are accessible for providers using Navina.
• Maintain confidentiality and verify consent forms in charts.
• Reconcile medications and coordinate appointments with care team.
• Communicate with patients via multiple channels to schedule visits and provide pre-appointment instructions.
• Document and log care management activities.
