Job summary

Silverado Hospice is hiring a Patient Care Coordinator for our Austin and Dallas offices. You will coordinate patient care, support clinical teams, and serve as a link between patients, families, and hospice leadership. This role requires living in Austin or Dallas and is not remote.

You will manage schedules, ensure compliance with regulations, and participate in IDG conferences while supporting compassionate, mission-driven care.

Qualifications

• Previous healthcare experience in an administrative setting required.
• Hospice experience preferred.
• Minimum of 1 year experience in a clinical healthcare setting (hospital, clinic, or home health agency).
• Reliable transportation to meet attendance and punctuality standards.

Responsibilities

• Maintain clinical responsibility for your team and act as liaison between the Director of Patient Care Services (DPCS), clinical associates, patients, families, and the community
• Manage clinical associates' schedules, including holidays, vacations, weekends, and daily assignments
• Investigate issues, notify DPCS for corrective action, and review hospice documentation for accuracy
• Participate in implementing clinical policies and procedures aligned with Silverado’s mission and goals
• Ensure compliance with local, state, and federal regulations and hospice standards
• Assist in planning and implementing hospice Interdisciplinary Group (IDG) conferences

We're hiring a Patient Care Coordinator for our Austin and Dallas offices– a dedicated and organized professional who ensures seamless coordination of patient care, supports clinical teams, and serves as a vital link between patients, families, and hospice leadership. Must live in one of the two listed territories: Austin or Dallas, this is not a remote position.

Qualifications

• Strong background in home health, healthcare, or hospice preferred
• Minimum of 1 year experience in a clinical healthcare setting (hospital, clinic, or home health agency)
• Reliable transportation to meet attendance and punctuality standards (driver's license not required unless specified)

Anticipated pay range: $25 USD - $30 USD
