Job summary

Silverado Hospice in Orange County is seeking a Patient Care Coordinator to ensure seamless patient care coordination and to support clinical teams. This role serves as a vital link among patients, families, and hospice leadership.

The ideal candidate is an LVN with state licensure and healthcare admin experience, with at least 1 year in a clinical setting, and reliable transportation to meet attendance standards.

Qualifications

• LVN with valid state license.
• Administrative healthcare experience required; hospice experience preferred.
• Minimum 1 year in a clinical healthcare setting.
• Reliable transportation to meet attendance standards.

Responsibilities

• Maintain clinical responsibility for your team and act as liaison between the Director of Patient Care Services (DPCS), clinical associates, patients, families, and the community.
• Manage clinical associates’ schedules, including holidays, vacations, weekends, and daily assignments.
• Investigate issues, notify DPCS for corrective action, and review hospice documentation for accuracy.
• Participate in implementing clinical policies and procedures aligned with Silverado’s mission and goals.
• Ensure compliance with local, state, and federal regulations and hospice standards.
• Assist in planning and implementing hospice Interdisciplinary Group (IDG) conferences.

What You’ll Be Doing

Schedule: Monday - Friday, 8:00am - 5:00pm

Anticipated pay range: $29 — $31 USD
