Job summary

Silverado Hospice is seeking a Patient Care Coordinator for the Austin and Dallas offices. This role coordinates patient care, supports clinical teams, and serves as a key link between patients, families, and leadership. Must live in Austin or Dallas; not remote.

You will maintain team clinical responsibility and ensure smooth operations in a mission-driven environment. Ideal candidates have healthcare admin experience, hospice exposure, strong organization, and reliable transportation for site

Qualifications

Previous healthcare experience in an administrative setting required.
Hospice experience preferred.

Responsibilities

Maintain clinical responsibility for the team and act as liaison between DPCS, clinical staff, patients, and families.
Manage schedules for clinical associates, including holidays, vacations, weekends, and daily assignments.
Investigate issues and review hospice documentation for accuracy and compliance.
Assist in implementing clinical policies and procedures aligned with Silverado’s mission and goals.
Ensure compliance with local, state, and federal regulations and hospice standards.
Plan and participate in IDG conferences.
