Silverado Hospice is seeking a Patient Care Coordinator for our Austin and Dallas offices. The role ensures seamless coordination of patient care, supports clinical teams, and links patients, families, and hospice leadership to maintain quality care and compliance.

You will participate in IDG conferences, manage staff schedules, and ensure regulatory adherence while upholding compassionate communication. Local candidates in Austin or Dallas preferred; remote work is not available.

Qualifications

• Previous healthcare experience in an administrative setting is required; hospice experience is preferred.
• Background in home health, healthcare, or hospice is preferred.
• Minimum of 1 year experience in a clinical healthcare setting (hospital, clinic, or home health agency).
• Reliable transportation to meet attendance and punctuality standards (driver’s license not required unless specified).

Responsibilities

• Maintain clinical responsibility for your team and act as liaison between the Director of Patient Care Services (DPCS), clinical associates, patients, families, and the community.
• Manage clinical associates’ schedules, including holidays, vacations, weekends, and daily assignments.
• Investigate issues, notify DPCS for corrective action, and review hospice documentation for accuracy.
• Participate in implementing clinical policies and procedures aligned with Silverado’s mission and goals.
• Ensure compliance with local, state, and federal regulations and hospice standards.
• Assist in planning and implementing hospice Interdisciplinary Group (IDG) conferences.
