Job summary

Silverado Hospice is seeking a Patient Care Coordinator to support the Austin and Dallas offices. You will coordinate patient care, serve as a liaison between patients, families, and leadership, and help ensure compliance with hospice standards. The role requires 1+ year in a clinical healthcare setting and strong organizational skills.

Qualifications

• Healthcare admin experience required; hospice experience preferred.
• Minimum 1 year in a clinical healthcare setting.
• Reliable transportation to meet attendance standards.

Responsibilities

• Maintain clinical responsibility for the team and act as liaison between the DPCS, clinical staff, patients, families, and the community.
• Manage clinical associates’ schedules, including holidays, vacations, weekends, and daily assignments.
• Investigate issues, notify DPCS for corrective action, and review hospice documentation for accuracy.
• Participate in implementing clinical policies and procedures aligned with Silverado’s mission and goals.
• Ensure compliance with local, state, and federal regulations and hospice standards.
• Assist in planning and implementing hospice Interdisciplinary Group (IDG) conferences.
• This position will also assume PCC duties for the Austin and Dallas offices.
