Americare Certified Special Services Inc. seeks a Patient Care Coordinator to organize all aspects of patient care, ensuring seamless communication between patients and the care disciplines assigned to each case.

The ideal candidate will have a high school diploma or GED, two years of recent home care or related experience, strong verbal and written communication, excellent organizational and customer service skills, and may be bilingual.
