Job summary

CareRing Health is seeking compassionate Patient Care Coordinators to join our homecare team. You will screen and onboard patients and caregivers, coordinate cases, and ensure timely, compliant service delivery for the patients and families we serve in Pennsylvania and Delaware.

The role emphasizes clear communication, reliability, and a dedication to helping those in need. Training is provided to new hires, with a supportive, collaborative work environment and opportunities for growth.

Qualifications

• High school diploma or GED required.
• Bilingual preferred.
• Excellent communication and organizational skills.
• Willingness to learn and adapt to training.

Responsibilities

• Screen new patients and caregivers
• On-board new patients, including providing assistance with any documentation/clearance requirements
• On-board new caregivers, including orientation/training
• Staff patient cases with caregivers and ensure patient cases are started promptly
• Ensure compliance with the law and Company policy, including caregiver clock-in and clock-out requirements
• Communicate with patients, caregivers, and patient families to ensure satisfaction and quality service delivery
• Assist with on-site visits, as needed
