Job summary

CareRing Health is seeking a Patient Care Coordinator to support patients and families in Pennsylvania and Delaware. You will screen and onboard patients and caregivers, assign caregiver staff to cases, and ensure timely, compliant service delivery while maintaining strong communication with patients and families.

The role emphasizes on-site visits as needed and training will be provided; bilingual skills are a plus, and a high school diploma or GED is required with higher education preferred.

• Educational background ranges from high school to college degrees.

Responsibilities

• Screen new patients and caregivers.
• On-board new patients and assist with documentation and clearance.
• On-board new caregivers with orientation and training.
• Staff patient cases with caregivers and start cases promptly.
• Ensure compliance with law and Company policy, including clock‑in/out.
• Communicate with patients, caregivers, and families to ensure satisfaction.
• Assist with on-site visits as needed.

Responsibilities

• Screen new patients and caregivers
• On‐board new patients, including providing assistance with any documentation/clearance requirements
• On‐board new caregivers, including orientation/training
• Staff patient cases with caregivers and ensure patient cases are started promptly
• Ensure compliance with the law and Company policy, including caregiver clock‐in and clock‐out requirements
• Communicate with patients, caregivers, and patient families to ensure satisfaction and quality service delivery
• Assist with on‐site visits, as needed
