Job summary

CareRing Health is seeking Talented Patient Care Coordinators to join our team in Delaware and Pennsylvania to help patients live happier and healthier lives.

You will onboard patients and caregivers, screen new patients, staff patient cases with caregivers, ensure timely starts, and maintain satisfaction with families through clear communication.

Qualifications

• Bilingual strongly preferred.
• Passion and dedication to help those in need.
• Strong work ethic and communication skills.
• No homecare experience required; training provided.
• High school diploma or GED; associate or bachelor’s degree preferred.

Responsibilities

• Screen new patients and caregivers.
• On-board new patients, including assistance with documentation/clearance requirements.
• On-board new caregivers, including orientation/training.
• Staff patient cases with caregivers and ensure cases start promptly.
• Ensure compliance with the law and company policy, including caregiver clock-in/out requirements.
• Communicate with patients, caregivers, and families to ensure satisfaction and quality service delivery.
• Assist with on-site visits, as needed.
