Responsibilities

• Obtain and update patient demographics and insurance information.
• Verify insurance coverage on date of service.
• Obtain patient signatures for forms and documents.
• Check patients in and out for appointments.
• Answer inbound and outbound calls.
• Schedule, reschedule, and confirm appointments.
• Process referrals and follow-up with specialists.
• Maintain patient records and electronic charts.
• Coordinate care across sites as needed.
• Communicate with patients, families, and healthcare professionals.
• Maintain compliance with HIPAA, OSHA and company policies.
• Cross-train and provide support at other locations.

Position: Patient Care Coordinator

Location: New York, NY 10010

Duration: 3 Month contract to start, potential for Extension/Conversion to FTE

Rate: $25/Hr

Description:

Patient Care Coordinator: This position is responsible for accomplishing various task that contributes to the organization success and patient satisfaction. The team the candidate will be working with typically consists of Providers, Medical Assistants, Nurses (RN/LPN), PSA, echo techs averaging 21 members daily. The team culture is characterized by high morals and strong interpersonal skills, which are essential for success in a fast-paced, environment. The candidate will be responsible for executing day-to day operations, ensuring compliance with regulations, and providing coverage across other sites as needed. Responsibilities:

• Obtain and update patient demographic and insurance information.
• Verify insurance coverage on the date of service.
• Obtain patient signatures for required forms and documents.
• Check patients in and out for appointments.
• Answer inbound and outbound phone calls.
• Schedule, reschedule, and confirm patient appointments.
• Process referrals and perform follow-up scheduling with specialists.
• Maintain work queues, inboxes, and pending patient actions.
• File, fax, scan, and maintain patient medical records.
• Monitor fax queues and assign documents to the appropriate electronic chart.
• Collect and process information in accordance with payment security guidelines.
• Coordinate care through scheduling and tracking systems.
• Communicate with patients, families, and healthcare professionals.
• Maintain HIPAA, OSHA, and company compliance standards.
• Work collaboratively with the care team in a fast-paced environment.
• Cross-train and provide support at other locations as needed.
• Perform additional duties as assigned.

Hours:

Mon-Thurs:8:30am-5:00pm, Fri: 8:00am-4:00pm EST

Min Education:

High School Diploma OR GED
