Qualifications

• Experience in home care or healthcare coordination preferred.
• Ability to manage a high-volume caseload and prioritize tasks.
• Strong communication with patients, caregivers, MLTCs, and internal teams.

Responsibilities

• Manage an assigned caseload of patients and oversee day-to-day coordination of services.
• Ensure patients are staffed with reliable caregivers meeting schedules and care plans.
• Address staffing issues to prevent gaps in patient care.
• Monitor utilization of authorized hours and staffing levels.
• Communicate with patients and representatives about status and concerns.
• Document communications accurately and timely; complete required reports.
• Escalate urgent patient, caregiver, or service issues to management when needed.
• Maintain accurate patient schedules and records in agency systems.
