HRforGrowth is seeking a Patient Care Coordinator in Long Island City, NY to manage an assigned caseload of patients and ensure consistent home care services. Location: Long Island City, NY. This onsite, full-time role offers $50K-$60K per year plus benefits.

The ideal candidate will have strong communication, organization, and problem-solving skills, with the ability to manage multiple priorities and maintain accurate documentation.

Qualifications

• Strong verbal and written communication skills.
• Excellent organizational and time-management abilities.
• Ability to manage a high-volume caseload under pressure.
• Experience in home care or healthcare coordination preferred.

Responsibilities

• Manage an assigned caseload of patients and oversee daily home care coordination.
• Staff patients with reliable caregivers meeting schedules and plans of care.
• Address staffing issues to prevent gaps in patient care.
• Monitor utilization and ensure authorized hours are staffed.
• Communicate with patients, families, MLTCs, hospitals, and payors.
• Document communications accurately and on time.
• Escalate urgent issues to management when needed.
• Maintain accurate patient schedules and records in the agency system.
