Responsibilities

• Identify immediate and long-range plans for patient care including patient and family in the plan
• Set priorities for patient needs reflecting sound judgement
• Monitor patient clinical status and assess for significant changes
• Communicate clearly with patient, family, physicians, and team members to ensure needs are met
