Job summary

HRforGrowth is seeking a highly organized Patient Care Coordinator to manage an assigned caseload of home care patients, ensuring staffing coverage and optimal utilization of authorized hours. You will coordinate caregivers, communicate with patients and representatives, and work with MLTCs, healthcare providers, and internal teams to resolve service-related issues.

This onsite role in Long Island City, NY offers a full‑time position with a salary of $50K–$60K per year plus benefits.

• Ability to follow up persistently and promptly until outstanding patient issues are fully resolved.
• Comfortable working with reports, scheduling systems, and patient management software.
• Previous experience in home care, healthcare coordination, customer service, scheduling, or a related field is preferred.
• Ability to maintain accurate documentation and records while meeting established deadlines.
• Strong interpersonal skills and the ability to build effective relationships with patients, caregivers, and internal and external stakeholders.

Responsibilities

• Manage an assigned caseload and coordinate day‑to‑day home care services.
• Ensure patients are staffed with reliable caregivers meeting schedules and plans of care.
• Address staffing issues to prevent gaps in patient care.
• Monitor utilization and ensure authorized hours are used.
• Identify patients with low utilization and take corrective actions.
• Maintain regular communication with patients or representatives regarding status and concerns.
• Communicate schedules and changes with caregivers.
• Coordinate with MLTCs and payors on services and authorizations.
• Monitor hospitalized or on-hold patients and resume services promptly.
• Follow up on missed visits, uncovered hours, and status changes.
• Collaborate with internal departments to resolve issues and complete reports on time.
• Document communications accurately and timely.
• Escalate urgent issues to management when needed.
• Maintain accurate patient schedules and records in agency systems.
• Build strong, professional relationships with patients and caregivers.
