Job summary

Aria Community Health Center is seeking a Care Coordinator to partner with patients and providers to promote timely access to care and continuity of care. You will identify gaps, assist with scheduling, and support navigation to ensure high-quality, preventive care.

In this role you will communicate with patients and families, coordinate non-clinical services like transportation, and document activities in the medical record.

Qualifications

• Associate degree preferred
• At least 1 year of case management experience
• Computer literacy required
• Cultural competency with the target population

Responsibilities

• Monitors Gap in Care reports to help schedule appropriate appointments
• Consults with patients and families to discuss health problems
• Plans non-clinical services such as transportation
• Documents client services in the medical record
• Plans and executes health events
• Supports patient discharge planning when needed
