Mercy Health in Warren, OH is seeking a Health Care Associate to provide direct patient care under the guidance of a Registered Nurse or other healthcare professional, following regulations and organizational policies.

Responsibilities include basic patient care, vital signs, hygiene, mobility, accurate documentation, and cooperation with the health care team. EPIC EHR experience and a current BLS certification are preferred; training and benefits are provided.

Qualifications

• Recent acute care, physician's office, home health/hospice, or long-term care facility experience preferred.
• BLS certification preferred upon hire.
• Must be able to follow care plans and report changes to RN.
• Demonstrates knowledge of patient age-specific care and safety practices.

Responsibilities

• Provide direct patient care under the guidance of the RN or designated professional.
• Liaise between patient and nurse to report changes/concerns.
• Deliver high-quality customer service to patients, families, visitors, and staff.
• Document patient care accurately and timely; participate in hand-offs.
• Perform delegated tasks once competency is validated.
• Assist with mobility and transfers.
• Encourage independence and personalize care.
• Adhere to infection control and safety policies.
