Job summary

Mercy Health in Warren, Ohio is seeking a Health Care Associate to provide direct patient care under the guidance of the Registered Nurse or designated health care professional. The role emphasizes compassionate patient support, accurate documentation, and teamwork within a busy clinical setting.

You will assist with vital signs, mobility, hygiene, and patient comfort, while communicating changes to the nurse and ensuring safety and quality care across age groups.

Qualifications

• Recent experience in acute care or long-term care preferred.
• Successful completion of nursing aide training OR STNA OR CNA OR nurse’s aide program; or equivalent clinical coursework.
• EPIC Electronic Health Record experience preferred.

Responsibilities

• Provide basic patient care including care, comfort, vital signs, hygiene, and mobility assistance.
• Act as liaison between patient and nurse to report changes and concerns.
• Deliver high-quality customer service to patients, families, visitors, and staff.
• Document patient care accurately and timely; participate in hand-offs.
• Perform delegated tasks once competency is validated.
• Assist with mobility and safe transfers.
• Encourage patient independence and participation in activities.
• Tailor care to age-specific needs and departmental policies.
