Job summary

NYC Health + Hospitals is seeking a Health Navigator to support NYC Care members through enrollment, scheduling, and portal usage. You will work closely with clinical sites and grant teams to promote access to care and telehealth services, while helping patients navigate MyChart and other digital tools.

The role requires strong customer service skills, comfort with technology, and the ability to interact with diverse populations. A healthcare-related degree or equivalent experience is preferred.

Qualifications

• High school diploma or equivalent
• Six months of full-time clerical/public-facing experience
• Associate’s or Bachelor’s degree in Healthcare Administration, Community Health, Public Health or related field; Master’s degree preferred

Responsibilities

• Provide tailored support to NYC Care members guiding enrollment, scheduling, attendance, and MyChart functionalities
• Keep records of all patient outreach attempts and outcomes
• Collaborate with clinical sites and the grant team to create educational materials and processes around telehealth and patient portal services
• Collect patient and staff feedback regarding experiences, barriers, and potential improvements in accessing care and patient portal usage
• Assist in improving current processes or grant activities to meet identified needs around access to care, telehealth and patient portal support
• Participate in special projects as needed and perform other duties as assigned
