Job summary

The Remedy in Minnesota is seeking a dedicated Patient Access Coordinator to support patient intake and scheduling. You will manage appointment scheduling, coordinate with providers, and help ensure smooth communication between patients and care teams.

The ideal candidate has strong organizational abilities, customer-focused attitude, and the capacity to work efficiently in a fast-paced environment. Two years of relevant experience and a high school diploma are preferred.

Qualifications

• High school diploma or GED required; customer service experience preferred.
• Two years of patient care coordinator experience preferred.
• Experience with scheduling and patient communication is desirable.

Responsibilities

• Answer incoming and make outgoing patient phone calls with exceptional customer service.
• Coordinate and schedule patient appointments with healthcare providers.
• Conduct initial patient screening and gather information to determine service type.
• Pre-screen new patients for initial assessment information.
• Answer inquiries, troubleshoot basic issues, and provide information to patients.
• Update and maintain accurate patient records in the system.
• Escalate calls to supervisor when appropriate.
• Obtain and accurately capture demographic and health insurance information.
• Utilize Outlook, Teams, 8X8, Advanced MD, Monday.com and other systems daily.
• Coordinate with other departments to ensure patient needs are met.
• Perform other duties as assigned.
