Job summary

SWLA Center for Health Services seeks a Patient Access Referral Coordinator in the Physician Referrals department to ensure confidential handling of patient referrals. The role ensures successful arrangement and completion of referral documents both internally and externally.

The person will manage referrals, coordinate with hospitals/providers, maintain tracking, and support HIPAA activities under supervision of the Site Operations Manager.

Qualifications

• Two years of post-high school training in office management or similar program.
• At least one year of medical terminology.
• Able to work without supervision.
• Understand English grammar and office etiquette.
• Clerical skills and ability to operate office equipment.
• Familiar with Microsoft Office.
• Organizational skills and ability to work under deadlines.
• Ability to multi-task, establish priorities, work independently.
• Clear speech and adequate hearing/vision for clerical tasks.

Responsibilities

• Answer telephone calls. Schedule appointments for SWLACH patients referred to other providers; follow up on referrals.
• Complete referral forms; mail/fax forms to providers and log into tracking system.
• Notify patients of appointments and answer questions.
• Maintain a record of outbound referrals and log into tracking system.
• Receive patient records/referrals and forward to Medical Records for scanning.
• Request additional information from providers by contacting hospitals/physician offices.
• Assist COO/Referral Supervisor with clerical activities and HIPAA tasks.
• Assume additional related functions as assigned.
