Job summary

Camino Health Center is seeking a Referral Specialist to join the population health team. You will process referrals, communicate with patients and providers, and manage scheduling and follow-up with both in-house and external specialists.

Under the supervision of the Population Health Manager, you will ensure timely authorizations, maintain HIPAA compliance, and support high-quality patient care through efficient referral workflows.

Qualifications

Responsibilities

• Process referrals from providers and communicate authorizations to patients and staff.

• Coordinate with care teams to ensure timely patient follow up and referrals.
• Communicate with patients and providers about referral details and next steps.
