Qualifications

• Must have a high school diploma or GED.
• Six (6) months in a fast-paced office environment meeting deadlines and proficiency in typing with accurate spelling and grammar.
• Basic Life Support (BLS) from the American Heart Association (AHA), must obtain within ninety (90) days of hire.
• Excellent customer service and telephone communication skills.
• Proficient knowledge and ability to use computer software.
• Positive communication and working relationships with others.
• Possession of high ethical standards and no history of complaints.
• Reliable and dependable; reports to work as scheduled without excessive absences.
• Ability to handle sensitive and confidential information.
• Ability to work independently with minimal to no supervision.

Responsibilities

• Participates in daily huddles to determine patient flow and care gaps.
• Serves as the clinic’s EHR super user and trains staff on software systems.
• Coordinates scheduling and referral management, health records maintenance, and data retrieval.
• Assists in empanelment by assigning patients to a PCP and ensures appointments with the PCP are scheduled.
• Oversees provider capabilities documentation and tracks patient panels for new patients.
• Maintains Health Information Exchange access and retrieves pertinent health data for providers.
• Keeps informed about clinical and hospital services to avoid delays in care.
• Completes assigned projects independently through analysis and troubleshooting.
• Manages internal referrals to avoid duplicates and supports improvement initiatives.
• Maintains clinic scheduling guidelines and wait lists in the EHR.
• Assists with Meds in Hand program, pharmacy pickup lists, and related tasks.
• Orders and requisitions to stock forms and supplies.
• Ensures proper PPE is worn and donning/doffing procedures are followed.
• Completes patient care documentation in the EHR in real time.
• Participates in workflow and project teams related to EHR or other initiatives.
• Performs other duties as assigned.
